IHI issued a challenge to every hospital in the country to help quickly spread the introduction of the World Health Organization (WHO) Surgical Safety Checklist, a tool that has shown major reductions in mortality and harm. The challenge is simple: Test the Checklist at least once with one operating room team.

As hospitals begin to adopt the Checklist, we are tracking the progress across the country on this interactive map on IHI.org. If you’ve tested the Checklist, or plan to do so, please let us know by completing this very brief survey.
