1. Introduction and Electronic Informed Consent to Participate

This research study is designed to better understand your experience with having a food allergic child attend a summer camp. You indicated an interest in participation in this study by clicking on a link from the web site of the Food Allergy & Anaphylaxis Network, a co-investigator in this study with the University of Michigan Food Allergy Center. This survey should take approximately 15-20 minutes to complete.

The information obtained from this survey will be anonymous and stored in a secure location. No single individual's identity will be linked to their answers.

Your participation in this survey is completely voluntary. The survey has been designed to skip participants around content that is not pertinent to their particular situation. We do not anticipate that one's participation in this survey would result in any risk to the participant. This study is survey based and does not involve any physical contact with participants. We simply are interested in your experiences as you cope with sending a child with a food allergy to a summer camp. The trends that we characterize from this study serve to potentially benefit food allergic individuals everywhere by introducing more awareness to how summer camps and parents of food allergic children attending summer camp manage food allergy. We hope this will make attending a summer camp safer for children with food allergies.

1. The following are a series of questions pertaining to food allergy and allergic reactions to food that have occurred at a summer camp. Most questions require either a "yes", "no", or "I do not know" answer. We do not ask for any personal information that would in any way identify you, the food allergic camper, or the camp. Participation in this study is completely voluntary. Data collected will be analyzed for present trends in food allergy preparedness among parents and camps. This information will be kept in a secure location, under electronic encryption, but we intend to publish these results in the Allergy/Immunology medical literature.
