The Utah State Chapter of the National Multiple Sclerosis Society is conducting a survey to better understand how multiple sclerosis has affected our members and to determine what programs will be the most beneficial to meet the needs of those individuals with MS and their families. We estimate the survey will take between 10-20 minutes. Your input is essential.

Your responses will be kept in the strictest of confidence. We are working with an independent consultant who will be reviewing the results and making recommendations as to how we can direct future programs, services and support in order to meet the needs of our clients. We will only add you to our mailing list or contact you directly if you have asked us to do so. If there are any questions that you would rather not answer please leave the question blank.
