The National Healthcare Decisions Day (NHDD) Initiative is a collaborative effort of national, state and community organizations committed to ensuring that all adults with decision-making capacity in the United States have the information and opportunity to communicate and document their healthcare decisions . The event is scheduled for April 16 and will be observed by many local organizations on or around that date.

The easiest and most common way to document your healthcare decisions is with an Advance Directive. This is a generic term for legal documents that describe your wishes for healthcare in the event that you are impaired by illness, injury, or medical treatment and unable to communicate those decisions yourself.
1. Do you believe an Advance Directive is a useful tool for communicating healthcare wishes?
2. Do you currently have an Advance Directive?
3. If not, what are your reasons for not having one?
4. If you do have an Advance Directive, have you given copies to family members or your physician(s), or HRMC?
5. Do you have aging and/or chronically ill family members that do not have Advance Directives?
6. Would you attend a one hour class after work sometime if the topic was Advance Directives, and you could fill one out as part of the class?
7. Would you be willing to bring family members and friends to such a class too?
