This program is funded by the CATIE Center at St. Catherine University under a grant from the Rehabilitation Services Administration of the U.S. Department of Education.

The purpose of this survey is to learn about your perceptions and evaluation of this advanced medical training. Responses will be used to evaluate the program and gather ideas for future programming like this. Your responses will be kept anonymous in reports of the evaluation data.
