1. Introduction and Consent
You are invited to participate in a research study on searching for Health Information Online. Our objective is to understand how people search for health information online and look for ways to support them doing this.
The risks associated with this study are minimal. It is not anticipated that you will experience any unusual amount of stress or discomfort as a result of participating in this survey. The benefits that may reasonably be expected to result from this study include the experience of participating in a user task scenario and helping to further knowledge about how people search for health information online.
Your participation in this survey will take approximately 10-15 minutes.
If you have read this information and have decided to participate, please understand your participation is voluntary and you have the right to withdraw your consent or discontinue participation at any time without penalty. You have the right to refuse to answer particular questions. Your individual privacy will be maintained in all published and written data resulting from the study.

Data collected in this survey will be used to develop a model of how people search online for health information. There will be no information collected to identify you. There is an option to submit your email address to enter a prize draw at the end of the survey, your email address will be stripped from the results before any data analysis takes place.
