Dear School Professional,
Thank you for your interest in taking the LIVESTRONG™ Survey of school professionals. The survey is being conducted by the Lance Armstrong Foundation (LAF). This survey asks questions about your experiences working with children who have been impacted by cancer.
Your answers to survey questions will help the LAF improve programs for school-aged cancer survivors and the school professionals who support them in returning to school after diagnosis and treatment. The results of the survey may also be used to educate other organizations and researchers about survivorship issues. These results may be presented in written and oral reports.
Your answers to the survey questions are anonymous. No one will be able to identify the specific information that you provide. Whether or not you participate in this project will not affect your present or future relationship with the LAF.
If you choose to take this survey, you will still have the right to change your mind. You can withdraw your consent at any time and no longer participate.
The full survey takes about 10-15 minutes to complete. You are not required to finish the survey. You do not have to answer all of the questions.
