Informed Consent

I understand that my participation in this project is voluntary and that I may withdraw from the study at any time without penalty. I also understand that the data will be anonymously coded to ensure confidentiality. The purpose of this study is to learn about important aspects of why people leave Christian fundamentalist churches.

As a participant in this study, I will spend approximately thirty (30) minutes completing questionnaires about topics such as religious beliefs, God image, spiritual development and motivation, and social support. This survey will not include my name or any other identifying information and will be stored in a secured location.

I understand that this research presents no risks other than what I might feel from thinking about the questions that are asked. I have a satisfactory understanding of what will be involved as to my participation. Any further questions about the research and my rights as a participant will be answered by contacting the project coordinator at fundamentalism85@gmail.com.

I have read the material above and I agree to voluntarily participate in this activity. Any questions I asked have been answered to my satisfaction prior to my completing the surveys.
