1. NSW Community Health Review

 
The NSW Department of Health has commissioned the Centre for Health Service Development to undertake a strategic review of Department of Health managed community health services in NSW. The review is looking for examples of good practice being used at the moment, and also for relevant literature or reports about community health practice and policy.

We are looking for feedback about community health services managed by the Department of Health - how they operate currently and what these services should look like in the future. There is a web-site - www.nswchr.net with more detailed information about the review.

This survey is designed for individuals who have limited time to engage with the review. All responses to this survey are completely anonymous. The results and comments of the survey will be posted onto the web-site and are intended to generate further discussion or submissions. Please ensure that you do not post anything that may identify you (unless you wish to!). Information drawn from the responses will be used to help us explore the themes for the review and will not be used to produce a separate statistical report.
