You are invited to participate in a research study about nonmainstream (extreme) body modification and its social implications.

You will be asked to answer twenty-one (21) questions in your own words. This will take about one hour of your time, depending on how detailed your answers are.

This is an opportunity for you to share the experiences, beliefs, and motivations behind your body modifications. By completing this questionnaire, you acknowledge that you will NOT receive compensation of any kind for your participation.

If you have decided to participate in this project, please understand that your participation is voluntary and that you have the right to withdraw your consent or discontinue participation at any time with no penalty. You also have the right to refuse to answer any question(s) for any reason with no penalty.

In addition, your individual privacy will be maintained in all publications or presentations resulting from this study. If you are interested in participating in future research and/or presentations, please include your phone number and/or email address. This information will not be shared with anyone!
