Thank you for participating in this survey.
The Center for Minority Health is conducting a brief research study on consumer preferences regarding food and grocery shopping, as part of a project to prevent diabetes and heart disease in Pittsburgh. We will be surveying people who live in different neighborhoods of Pittsburgh, and asking them to complete a brief (approximactely 15 minute) questionnaire.
This is a survey adapted from one made by the Consumer Health Coalition, Inc., Los Angeles; additions include questions devised by the Cancer Prevention Research center at the University of Rhode Island.
I want to let you know that your responses to these questions are confidential and we will not ask for your name as part of this survey. If you are willing to participate, our questionnaire will ask about background (e.g., age, race, neighborhood of residence), as well as about your feelings about your grocery shopping experience and the grocery store you frequent. There are no foreseeable risks associated with this project, nor are there any direct benefits to you.
You will receive a gift from CMH as a token of our appreciation. Participants at Kingsley and Hosanna House will be able to pick up their gift immediately; participants at the university are requested to print out the thank you page that appears at the end of the survey indicating that you completed the survey and bring it to the CMH to receive your gift.
Your participation is voluntary, and you may refuse to answer questions in the survey that you do not wish to answer. This study is being conducted by Center for Minority health at the University of Pittsburgh, who can be reached at 412-624-5665, if you have any questions.
