2011 Exit Interview for the State of Colorado

Thank you for taking the time to complete this survey concerning your departure from employment with the State of Colorado. The purpose of this survey is to gather data on why people leave employment with the State of Colorado. Your candid comments are greatly appreciated. Data may be shared with Human Resources Directors, but will not be linked to you or the date the survey was taken, nor be placed in your personnel file. To assure this, we ask you not to provide your name and contact information. In addition, completion of this survey is not part of any grievance process, nor does is constitute official notice of violations of federal or state law.
