For my doctoral research, I am conducting a survey of affiliate marketers. Results of the survey will be published in my dissertation and available to survey participants upon request.

The purpose of the first section of this survey is to gather demographic data that is needed for overall analysis. The second section contains multiple choice questions regarding Google AdSense/AdWords. The third section asks you to indicate the specific statement that most accurately reflects your opinion. The final portion of this survey contains open-ended questions about Google AdSense/AdWords. All responses will remain anonymous.

Informed Consent Preamble

Please read this consent agreement carefully before agreeing to participate in this study.

You have been invited to participate in a study being conducted by Woradech Na Krom at Argosy University, Southern California to determine the relationship between individual attitudes and affiliate marketing practices online.

What you will do in this study: Participation in this study consists of completing the questionnaire on affiliate marketing beliefs and practices. This involves answering a series of questions regarding demographics, your beliefs, opinions, and practices online.

Time required: The questionnaire will take approximately thirty minutes to complete.

Risks: There are minimal risks for participation in this study. Participation will remain anonymous and no personal data will be published. This survey is designed to examine the relationship between beliefs and practices online in the aggregate and will not be linked to any one individual. However, you may feel some emotional discomfort when answering questions about your personal beliefs or practices.

Benefits: There are no direct benefits to participants. However, it is hoped that your participation will help researchers learn more about the effect of attitudes on Internet marketing behavior. It is anticipated that this knowledge will be beneficial to e-commerce and consumers in general. A copy of study results will be sent to you upon request.

Confidentiality: All information provided will remain confidential and will only be reported as group data with no identifying information. All the information gathered from the study will be kept in a secure location and only those directly involved with the research will have access to them. After the research is completed, the information will be destroyed after a period of a year.

Participation and withdrawal: Your participation in this study is completely voluntary. You may withdraw from the study at any time without penalty by deleting your survey responses or requesting that they be deleted. You also may withdraw by e-mailing Woradech Na Krom stating that you no longer wish to participate and your survey will be deleted if it has already been collected. Any personally identifiable information will be deleted simultaneously.
