Hudson Valley Region Health Assessment

Thank you for taking the time to complete this survey about the health of our Hudson Valley region. The County Health Departments including Dutchess, Orange, Putnam, Rockland, Ulster, Sullivan and Westchester along with New York Medical College are in the process of conducting an assessment to guide planning efforts over the next two years to improve the health of the region.
