INFORMATION SHEET FOR PARTICIPATION IN RESEARCH STUDY
NURSES' PERCEPTIONS OF CHANGE OF SHIFT REPORT
You are being asked to participate in a research study conducted by Maggie Freeman, a graduate student at DePaul University, as a requirement to obtain her Masters degree. We are asking you because we are trying to learn more about nurses' opinions regarding change of shift report. This study will take about 10 minutes of your time. If you agree to be in this study, you will be asked to fill out the following survey. The survey will include questions about your demographic information and your opinions and views regarding the different types of shift report. You can choose not to participate. There will be no negative consequences if you decide not to participate or change your mind later.
In order to participate, you must be fluent in English, and a current nurse, former nurse, or nursing student with some floor experience.
