This is the Adult fare trial sign-up request form.

*Senior, Youth and Disabled customers please do not fill out this form. You may sign up to join the participant group or receive a free card by entering your information at translink.org/muni using the Senior, Disabled, Youth Customers tab located on the left side of that page.

Please note: TransLink is accepted on BART, Caltrain, AC Transit, Dumbarton Express, Golden Gate Transit and Ferry and all of Muni except for cable cars.

5. As a participant in SFMTA's TransLink on Muni Adult Trial Group, I understand that:

-TransLink is currently not available on cable cars. I will not be able to use my TransLink card as fare payment on this service.

-A Fast Pass loaded onto my TransLink card cannot be used fare payment on cable cars.

-I am helping test the system for technical issues prior to TransLink's availability to the general public.

-I need to report any issues or problems with using my card to the TransLink Customer Service Center.

-If my card or the system doesn't work, I need to confirm with the operator and may be required to provide alternate fare payment.

-I will periodically be asked to fill out surveys that relate to my commuting experience using TransLink.

-I cannot punch a hole in, alter or bend my TransLink card. Doing so will damage the card and I will be responsible for any fees related to replacing the card and restoring the balance.

-If my card is lost or stolen, there is a $5 fee for replacing the card and an additional $5 fee to restore the balance. If I am registered for Autoload, the card replacement fee will be waived and I will only be responsible for the $5 fee to restore its balance.
