Welcome and thank you for volunteering to participate in our research study.

The purpose of this study is to bridge the gap between parents and professionals regarding the treatment of children with autism. We are looking at the effectiveness of Occupational Therapy (OT) at increasing participation in children with autism. Three treatment types, Applied Behavioral Analysis, Cognitive Behavioral Therapy, and Sensory Integration, are being compared based on parental observations. Please take a moment to fill out the survey for this purpose.

The survey is 60 questions and should take no more than thirty minutes. Please answer each question to the best of your ability. Your opinion is very valuable to this study. There are very minimal risks associated with your participation in this study. Specifically, you may feel fatigued after completing the survey.

The benefits of participating in this survey is that you would be helping the Occupational Therapy profession evaluate the effectiveness of intervention styles, with the hopes of providing the most effective treatment for children with autism spectrum disorders. This information may benefit your child in the future as well as other children with autism.

All of your answers on this survey will be kept completely confidential. You will be asked to give information about your socioeconomic and marital status as well as your age, gender and ethnicity. However, you will not be asked to give any identifying information such as your name or address; as a result, information obtained will be anonymous. All hard copies of this data will be stored in a locked filing cabinet in the offices of the investigators.

Again, thank for your participation in this study, and we would like to remind you that your participation is voluntary. You may exit this survey at anytime with no penalty. If you choose to exit the survey early, your data will not be submitted. By submitting this survey, you are agreeing to the use of this anonymous data for educational purposes by the students of Columbia University, their faculty supervisors, and the original creator of the survey, Gary Bedell, Ph.D., OTR.
