Chronic Disease Teleseminar - Registration Form

Please sign up below for the CMHA Ontario teleseminar on Mental Health, Mental Illness and Chronic Disease. The seminar will be offered twice, on February 11 and February 26, 2009. Please indicate your choice below.

Please note: Registration is limited to 25 sites per event. First come, first served. We request that only one telephone line be used per site. If you are registering on behalf of everyone in your organization, please indicate below how many people will be participating.

Event format: There will be a 20-minute presentation, followed by a discussion. The presentation will be e-mailed to you ahead of time.
