Written Materials: Upon receipt of this survey, we will send you a Green-Pink Party Kit containing information about women’s health and the environment. We encourage you to share this information as freely as you wish, but ask that you do not reproduce any of the materials without the Green-Pink Partnership (Sierra Club & One By One) contact information attached. If you plan to publish additional written or website promotion for your party, we ask that you provide copies of the proposed materials to us for review at least seven [7] days before your publishing deadline.

Recordkeeping and Receipts: By accepting these terms of agreement, you agree to provide the Green-Pink Project with the amount of each contribution received and the name and contact information, including email address wherever possible, of each contributor. (Note: The Green-Pink Project does not share this information with any other organizations.) Please forward all contributions to One By One (4041 Roosevelt Way NE, Suite C, Seattle, WA 98105) within 7 days of receipt. Upon receipt of the checks and written information described above, One By One will send a thank you letter/tax receipt to each donor.
