The purpose of this survey is to evaluate the Asthma in Maryland 2007 surveillance report by

(1) Obtaining information on how the surveillance report is used and by whom,
(2) Assessing the usefulness of the surveillance report, and
(3) Assessing whether changes in the annual surveillance report are needed.

The information obtained from this survey will be utilized by MACP to improve asthma surveillance activities in Maryland.
