As Outreach magazine gears up for our second annual America's Most Innovative Churches list, coming in the January/February 2008 issue, we want your help! We want to hear about the churches YOU think are innovative, the diverse churches, perhaps under our radar, that are doing ministry "out of the box." It could be your own, or one you admire. We invite you to nominate this church for consideration by our panel of experts as they compile a list of 25 churches, both from your nominations and their own research and experience, that will be featured as America's Most Innovative Churches in 2008.

NOTE: You are not "voting" for which churches make the Most Innovative Church list, so it doesn't matter how many times a certain church is nominated. The purpose of this survey is to suggest innovative churches that will be evaluated along with the OTHER churches our panelists are considering. A nomination does not guarantee a church will appear on the list.

Innovation comes in many forms -- technology, media, community involvement, unconventional ministries, unique service styles, and much more. Please use the following list as a guideline to detail the kind of innovation that is happening at the church you're nominating. Check "yes" if the church is doing that particular innovation at this time. Check "no" if the church is not doing that innovation at this time. Please add any comments to help the panel consider your submission.

Our panel of 12 ministry leaders and experts--some of the leading voices in church creativity and the marketplace today--will consider your nominations in addition to their own as they compile our second annual list of America's Most Innovative Churches.
