Local Review Committee Partners Satisfaction Survey

In an effort to improve the quality of behavioral services in the state of Florida, Local Review Committees were established. We are making efforts to assure that the Local Review Committees remain a helpful and important resource for improving the quality of behavioral practices in the State of Florida. One of the ways to assure that this occurs is to solicit feedback from the “customers” of the Local Review Committee.

This survey is one of the ways we are attempting to increase feedback regarding the performance of each Local Review Committee. Please take the time to complete this survey each time you attend a Local Review Committee.
