Thanks so much for being willing to take the survey! I hope you really enjoy my thank you gift, an audio of my class, "The Stress-Fat Connection". This survey should only take 5 - 7 minutes of your time and will help me provide you the information and support you really want.

I appreciate your help. All answers will be held in the strictest confidence and not viewed by anyone other than myself.

This first page of questions (there are only 3 pages) will help me create a better newsletter based on your needs and interests.

1. What possible newsletter categories would you be most interested in seeing in Healthier Outcomes' "Stop the Weight!" newsletter? (mark all that apply)

2. In the past I have included articles and/or information that are relevant to entrepreneurs and business people. I have done this knowing that many of my clients have been entrepreneurs and health and fitness is a vital aspect of a thriving business.

Are you interested in continuing to see this information occasionally in the newsletter?

3. Stop the Weight! is currently a monthly publication. I am considering increasing the frequency in order to bring you more timely information and in smaller amounts so the newsletter is not so long as it is as a monthly.

What is your preference for receiving the newsletter? Keep in mind the more often, the shorter the length. (select as many as apply)

4. Currently all the information in the newsletter can be read within your email program, if desired. For a cleaner look, I can include the first paragraph or so of an article and include a link to click through to the entire newsletter that can be read on the web.

Would you be just as likely to read the newsletter if you had to click through to the web to read the entire article?

5. I've taken on a crazy challenge to write 100 articles in 100 days! I have ideas of things to write about but would prefer to write about what you really want to read. These articles will end up on my blog, website, and most importantly in the newsletter.
