Welcome to the Cyberchondria survey!

This is a shortened version of a longer survey given to subjects with Heightened Illness Concern. There are 19 items to answer and will take about 5-10 minutes of your time.

Please understand that the information you provide will be kept confidential by the research staff at the Columbia University Heightened Illness Concern program. We will not collect any identifying information from you, we will not contact you [unless you contact us first by email (illnessconcern@aol.com) or phone (212-5435542)], and we will not collect any computer information such as an IP address or URL.
