Research consent is provided to all individuals whose data may be used as part of a research project, no matter what part of the study they participated in .
We are conducting this research project to determine the impact of the UNH $3.13 A Day Food Challenge Initiative. We are writing to invite you to participate in this project by completing the web based surveys. You do not have to complete any of the surveys to be part of the 3.13 A Day Food Challenge, but your in-put will help us understand how this Challenge impacted our community. As part of the 3.13 A Day Food Challenge, you will have the opportunity to provide information in a variety of ways.
1. At sign up, a pre survey for the UNH $3.13 A Day Food Challenge will help us identify who is participating.
2. During the challenge, you are invited to join an ongoing discussion blog regarding the challenge.
3. You are invited to keep your own food log and submit electronically. You may complete on a daily basis, or at the end.
4. Completing the final survey after the campus sponsored UNH $3.13 A Day Food Challenge will help us understand the full impact of this project (see below) .
All you are asked to do when you agree to provide information is to allow us to report the findings from these assessments. Participation is strictly voluntary; refusal to use any of the assessment strategy tools does not preclude your joining the UNH $31.3 A Day Food Challenge. You will not be coerced into active participation in the UNH $3.13 A Day Challenge if you prefer to remain a non-participant. The potential risks of participating in this study are minimal. You will not receive any compensation to participate in this project. The anticipated benefits if you decide to participate in the $3.13 A Day Food Challenge are those experiencing a glimpse of the daily struggles many Americans face to secure food. All participants who agree to participation via the sharing of their data will be entered into a food basket raffle worth approximately $30.00 each. Three food baskets will be given away upon completion of the project.
We seek to maintain the confidentiality of all data and records associated with your participation in this research. There are rare instances when we are required to share personally-identifiable information (e.g., according to policy, contract, and regulation). For example, in response to a complaint about the research, officials at the University of New Hampshire, designees of the sponsor(s), and/or regulatory and oversight government agencies may access research data. You should understand that any form of communication over the internet does carry a minimal risk of loss of confidentiality.
Number codes will be used instead of names in the electronic files. The code sheet will be kept in a locked file in the Dietetics office. The work will be conducted by the Director of Dietetics at UNH, six UNH dietetic interns, and Juliet Bluemling, a Hospitality Management student.
