You could win $100 by returning this survey!
You may fill out the survey anonymously, but if you want to be eligible for the prize drawing, you MUST give us your contact info at the end of the survey so we can reach you if you win. Your name, personal information and answers will remain confidential. We will select three names at random to receive prizes.
