I hope that it won't take too long to fill in this questionnaire and I've tried to keep the questions to a minimum in order that I don't disrupt your busy lives too much.

I am issuing this annual survey for SNAG members to complete. SNAG is evaluating its work and building up a valuable evidence base about what it does (for funding bids etc.), establishing need for the service through feed back from its members.

Any personal information you provide will NOT be shared with anyone else unless you give your permission for me to do so. SNAG will keep this information for its own records and it will be used to improve the service SNAG provides.
