Informed Consent
My name is Ryan Cheperka and I am a graduate student in counseling psychology at Southern Illinois University, Carbondale. I am asking you to participate in my research study examining self-identified lesbian women’s attitudes about relationships and towards bisexual women.
Participation in this study is voluntary and you may withdraw at any time without penalty. Completion of the survey will take approximately 5 to 10 minutes. You will be asked to answer a number of questions relating to relationship values, attitudes, and expectations; as well as questions regarding bisexual women. Your responses will remain confidential and will be available only to the researchers who will have access to the data through the surveymonkey.com server. Identification codes, rather than names, will also be used to assure your anonymity. Upon full completion of the survey, you will have the option of submitting your name and e-mail address to the researcher. This will allow you to enter into a drawing for a chance to win one of three $15 gift certificates to Barnes & Noble. In other words, one in every 86 participants will win a gift certificate. Your name and email will be in no way connected to the responses that you give for the survey.
