2009 Sandoval County Community Health Survey

The Sandoval County Health Alliance works to improve the health and quality of life in Sandoval County through coalition building, health assessment, and community action. The Alliance is currently updating the Community Health Profile to identify and map our community's health resources, needs and characteristics. We will use the profile information to prioritize needs and develop strategies for change. By taking about ten minutes to complete this survey, you will help us identify community priorities.

WE WOULD LIKE THE INPUT OF COMMUNITY MEMBERS, MEMBERS OF COMMUNITY ADVISORY AND ADVOCACY GROUPS, AND PUBLIC AND PRIVATE EMPLOYEES OF ORGANIZATIONS WHICH SERVE COUNTY RESIDENTS.
