1. Description of the Beatrice D. Wade Award of Clinical Excellence

 
The awards committee at the Department of Occupational Therapy at UIC is seeking nominations for The Beatrice D. Wade Award of Clinical Excellence. This award is given annually to graduating occupational therapy students who have shown an exemplary commitment to occupational therapy during the academic program, and in particular those who have demonstrated outstanding abilities during their fieldwork experiences. The recipients of this award are honored at the UIC Occupational Therapy Pinning Ceremony, and they also receive a monetary award.

If you would like to nominate a UIC OT student for this award, please complete this survery OR submit a nomination letter with your rationale for nominating the student. It is important that this survey OR your letter reflects performance beyond entry-level expectations, as the committee is looking for exemplary students.

The committee uses the following documentation to select awardees:

1. Nomination has been submitted by fieldwork supervisor(s) and reflects the student’s fieldwork performance that is beyond entry-level competence.

2. The student demonstrates a commitment to the profession of occupational therapy.

3. The student is in good standing in the OT curriculum, with a minimum cumulative GPA of 4.0 on a 5.0 scale.

4. The student nominee has submitted a letter in support of the nomination.
