1. Consent for Online Screening Questionnaire

This questionnaire form will help us determine your eligibility for our study on the experience and physiology of emotion. It should take about 3-5 minutes to complete. Completing this questionnaire does not guarantee that you will be enrolled in the study. Some people might find these questions personal or sensitive. Your answers will be completely confidential and will not be linked with your name and contact information on any records.

The data will be destroyed if you are not enrolled in the study, and retained anonymously if you are enrolled in the study. You do not have to answer any of these questions. However, if you skip them, you will not be eligible to enroll in the study.

Note: If you have already participated in the UCB Emotion Study (i.e., you have already received a qualification email from ucbemotion@gmail.com), you will not be eligible to participate again. Please e-mail ucbemotion@gmail.com if you have any questions about this.
