THIS IS A SURVEY FOR THE SENIOR CLASS IN ALL URBAN ASSEMBLY HIGH SCHOOLS.

This is NOT a test.

There are NO wrong answers. We want to know what YOU think about school and your future plans.

Your answers are CONFIDENTIAL. They will be combined with all other students' answers in different schools to get a better idea about this year's Senior Class.

That means that no one can match your answers directly to you.

The purpose of the survey is to see how Urban Assembly high schools are doing, and how they can do better.

This survey is VOLUNTARY.

You do NOT have to answer any question you do not wish to answer, but we hope you will answer as many as you can.

There are 35 queestions and this survey should take less than 15 minutes.
