This survey is targeted to adults with developmental disabilities who participated in Michigan's Transportation Voucher Program any time from 2005 to 2008. In order to complete this survey, you must answer "Yes" to BOTH of the questions below:

I participated in Michigan's Transportation Voucher program any time from 2005 to 2008.
