Should I speak up or shut up?

I was completely swamped the other day and may have made a mistake with the dosage of medicine I gave a patient. Since he didn’t have an adverse reaction, I didn’t tell the doctor or him about my possible error. (I didn’t want to be written up.) Now, the patient’s kidney function has declined, and I don’t know if it’s because of what I might have done. What should I do in this kind of situation?
