This survey is designed to collect data regarding AAPT/PTRA Institutes held around the United States over the past 10 years. We realize it may be difficult for you to answer all of the questions if it has been awhile since you attended. Please answer all questions as honestly as possible. Since the survey is critical to our evaluation process, we want to give you as much opportunity as possible to complete the survey. Therefore, if you need to leave the survey, you may do so and re-enter at the point you left off as long as you use the SAME computer (and no one else takes the survey before you finish on that computer). The estimated time for completion is between 45 minutes and one hour.
