In 2007 "The Tradition Began"... In 2008 "The Tradition Continued", for our third season we had "A Tradition of Excellence". Now, as we plan for 2010, we REALLY need your help to improve your Patriots program. Please provide your honest and thoughtful feedback - positive or negative - on this three page survey. Part One asks about your coach(s). Part Two focuses on the organization. Part Three is for your child to fill out. Please know that your responses will be confidential unless you choose to include your contact information. IMPORTANT - IF YOU HAVE MORE THAN ONE CHILD IN THE PROGRAM, PLEASE COMPLETE SEPARATE SURVEYS FOR EACH CHILD.
