Neighborhood Place brings many publicly funded health, education and human services programs together under one roof. Each Neighborhood Place Center serves an area within specific boundaries. Residents, service providers and others help to guide these eight centers through Community Councils.

Please provide information as indicated in this application. Most responses are required in order to move forward and complete the form. If you choose not to complete the form, you may exit the application at any time.
