Thank you for participating in the Working with Youth Study. We hope you will help us understand more about what makes you an effective youth worker.

If you choose to participate, you will be able to complete the survey by following the simple instructions provided. There are a few things you need to know before starting:

- The survey will take approximately 20 minutes of your time and ask about the program you work in, your skills, and your professional experience.
- Once you have started the survey, you will not be able to return to complete it at a later time. Please complete it before logging out.
- If possible, please complete in a secure location so that others will not be able to view your responses.

The survey does not ask for any personal identifying information, and thus is anonymous. Your responses will be kept confidential and your participation in this study is completely voluntary. You can withdraw from participation at any point and choose not to answer any question(s) if, for example, any make you feel uncomfortable.

As a thank you for your participation, you will be given the opportunity to enter a raffle for a $50 gift certificate. A total of ten $50 gift certificates will be awarded, which can be redeemed at Amazon.com for program materials (for example art supplies, books, media materials). If you decide to participate in the study and choose to enter the raffle, you will be given an email address upon completion of the survey. Send an email to our office indicating that you want to be entered in the raffle. The winners of the prizes will be selected from the emails. In order to send out the prizes, a prize announcement will be sent out via email requesting contact information. All this data will be protected on a secure server. After the prizes are sent out, the list and information will be destroyed. The study will be open for approximately 6 months. Thus, the longest total time a participant may wait after entering to receive a prize will be approximately 7 months.

While the website where the online survey is posted has an excellent security system, the absolute protection of data transmitted over the Internet cannot be guaranteed. Also, the survey website does require that your computer be set to allow 'cookies' to insure that you can move from page to page, however IP addresses will be omitted from data reports before data are downloaded from the survey site to insure anonymity.

The data collected will be used to examine the relationships highlighted above. A report on the findings will be distributed nationally. Again, individual participants will not be identified in the data or subsequent findings.
