1. Study Purpose and Process
Welcome to the LEO-HP-NC Study, sponsored as a free service to NC law enforcement officers and their agencies by The FMRT Group (www.fmrt.com). By participating you can contribute significantly to the health and welfare of individual officers and their agencies in North Carolina.
Purpose: LEO-HP-NC is designed to collect ongoing information about the work, health, performance, and quality of life of NC law enforcement officers.
Procedures: If you agree to participate, you will be sent an email each quarter directing you to a secure website to an on-line survey. Each survey should take about 10-20 minutes to complete. You may choose to participate for one or more years or you may discontinue your participation in the study at any time.
If we lose contact with you, we may also try to contact you by an alternative contact that you designate by phone or mail. Therefore, should you wish to stop your participation, please notify the study contact via email of your intention to do so, so that we stop efforts to reach you.
