Dear Parent,
Please help us to help your child by completing this anonymous survey. Your participation is very important!
The SPD (Sensory Processing Disorder) Foundation is a Colorado non-profit public charity dedicated to SPD research, education, and advocacy. Currently, we are studying issues related to the diagnosis and treatment of SPD, as well as the costs incurred by families of children identified with the disorder.
It is our goal to share this information with professional diagnostic committees in order to demonstrate how important it is that SPD is represented in a broad variety of medical and mental health manuals. Parent/child experiences regarding diagnosis and treatment of SPD are a very important part of this process.
Before you begin this survey, please gather information regarding diagnoses you've received for your child, medication that has been prescribed, as well as cost information for doctor visits, medication and treatment.
Please respond to this survey with one child in mind. Once you complete the survey, you may re-enter the site and complete multiple surveys for additional children.
