This evaluation is to be completed by everyone seeking CEUs for attending the Symposium. Please complete the evaluation no later than March 7 in order to receive your 6 hours of CEUs. It is not necessary to enter your name, so your evaluation may remain anonymous if you so choose. We must receive at least 90% of evaluations completed or we will not be able to issue CEUs.
