Welcome to the Cal Poly Pomona Consumer Health class student designed Consumer Health Assessment.

To help the students learn about consumer health and its relationship to health behaviors and health status, please answer the questions that start on page three. The assessment will take about 20 minutes to complete. Your answers are confidential and anonymous. You do not need to answer questions if you prefer not to answer them.

For best results to assist the students answering all questions to best of your abilities will be most helpful. The correct answers with at least one reference are shown on the next page just before the next question. All the questions and more detailed explanations are on the web page you will be taken to upon completion of the entire assessment
