You are cordially invited to take part in a study designed to examine the relationship between acceptance of disability and life satisfaction in people with progressive disabilities.

The survey contains three main sections: consent form, demographic form, and questionnaires. There are 5 pages to the survey with a total of 83 questions, which will require about 15 minutes to complete.
