The concepts listed in this survey are commonalities for both private and public sector organizations that tend to produce excellent performance over time. Under each broad concept are specific practices that allow the concept to flourish.

Complete the survey by first reading the broad concept. Next rate your organization in terms of each of the four practices associated with that broad concept. Your "organization" may be defined as a single building, department, agency, or the entire system. Check with the survey administrator if there is any doubt about which level you should focus on while completing the survey. The survey will be of the most value in focusing school improvement efforts if all who participate are candid with their responses. Please be assured that your responses will remain anonymous.

Mark 1 (low) if you believe the practice is very weak or rarely practiced. Mark 5 (high) if you believe the practice is very strong and is almost always practiced. Mark 2, 3, or 4 if you believe the practice is somewhere in between. Mark N/A if you have no basis to judge.
