Tampa Bay Regional Health Organization (RHIO): Health Information Technology and Cosumer Use Survey
Computers and the Internet are a growing part of our lives. They are playing an increasing role in health care as well. We want to work with you to learn how computers and the Internet can help to improve your health.
This survey was developed by the University of South Florida in collaboration with the Tampa Bay Partnership. It will help all of us better understand how people in our community use health information and how computers and the Internet can play a role to improve quality and safety in health care.
Your organization has volunteered to help us obtain this information by advertising this survey through their internal distribution system. We ask if you are at least 18 years of age that you take about 10 minutes to fill it out.
THIS SURVEY IS ANONYMOUS. NOTHING YOU TELL US WILL BE LINKED TO YOU PERSONALLY. NO INDIVIDUAL ANSWERS WILL BE REPORTED. ONLY AGGREGATED OR COMBINED DATA FROM THE ENTIRE SURVEY WILL BE REPORTED. We do ask for your zip code and city (neither are required to complete the survey) so that we can know generally where responses came from in the Bay Area.
