This survey is for all Ohio public libraries that offered a Summer Library Program / Summer Reading Program (SLP/SRP) in summer 2019, regardless of the theme used or ages served. If you represent a library system, please include the numbers/results from all your branches or locations in one survey. 

Your responses will help the State Library, Regional Library Systems, other partners, and the Collaborative Summer Library Program to provide helpful resources and support for future SLPs/SRPs. We are no longer asking libraries to submit registration and completion numbers, because an increasing number of libraries no longer use traditional registration or completion methods.
To obtain meaningful statewide numbers, we are just asking for the number of individuals who participated in your SLP/SRP as you define participation, and the head count at SLP/SRP-related programs.
If you do register individuals and track completion, you are welcome to describe these results in question #7.

We welcome suggestions on any aspect of SLP/SRP support/resources. 

We also encourage you to share a narrative anecdote about your SLP/SRP or related activities, such as community partnerships or SFSP participation. Specific examples of the positive impact of your summer activities on a child, teen, adult, or family in your community are particularly valuable. You are not limited to the visible space below - share as much as you care to!

* 14. Do you permit the State Library to include quotes from your survey response in the narrative report of the 2019 Ohio SLP/SRP Evaluation? This report will be used to tell the story of summer library programs around Ohio and how they positively impact children, teens, and communities.
