YOUR CHANCE TO TELL US WHAT YOU NEED FOR YOUR FUTURE END OF LIFE PLANNING

COTA Victoria and Transgender Victoria, in conjunction with the National Ageing Research Institute are undertaking a survey of older people (aged 55 years and over) living in the community who identify as LGBTI. The research project aims to collect information regarding community needs and current practices, if any, about end of life care and planning.

The National Ageing Research Institute (NARI), an independent not for profit research institute has been funded by COTA Victoria to oversee this project.

The information will be used to help COTA Victoria and Transgender Victoria develop information and education resources to raise awareness and help older LGBTI people in their end of life care planning.

Please note:
  • You do not have to complete this survey if you do not want to
  • Your participation is completely voluntary
  • Fill in the questions you feel comfortable answering
  • This survey is anonymous.  The researchers have no way of identifying who has completed each survey and it will not be possible for you to be identified in any report or publication of this work
  • By completing the survey online you are giving your consent to take part in the survey and for the researchers to compile reports/publications of the results 
  • This survey can be printed out and done by hand and mailed to the associate researcher, Myfan Jordan, Level 4, Block Arcade, 98 Elizabeth Street, Melbourne VIC 3000
There are 23 questions which relate to the research and 2 others if you would like to get involved - it should take no more than 15 minutes to complete.
