1. APMA Insurance Complaint Survey

The insurance complaint survey is designed to assist APMA members in reporting problems they encounter with both private and public insurance carriers. It also serves as a mechanism to better track key issues and concerns affecting the podiatric community.

This survey enables members to use the APMA website to relay information directly to Health Policy & Practice staff so that we may assist you appropriately regarding key reimbursement matters.

Complete this survey if you are unable to address your insurance matter, either individually or with the assistance of the CAC/PIAC representatives and/or your state component. We encourage all members to fill out this survey entirely.

This survey will help us better assist you in dealing with your insurance complaint and or concern. Identifying areas of concern for our members remains a top priority of APMA staff.
