Nomination Form

This award acknowledges a child or young person from one of our Outside School Hours Care (OSHC) services who aspires, has reached achievements (great or small) and most importantly, inspires.
 
Nominations open Wednesday 12 May 2021. Nominations close 5pm Friday 11 June 2021.

Who can nominate?
We welcome any Y staff or parents/care givers of a child or young person attending one of our OSHC services to nominate and recognise.

The nominated child or young person must attend one of our Y WA Outside School Hours Care (OSHC) services to be eligible.

The nominated child or young person must attend one of our Y WA Outside School Hours Care (OSHC) services to be eligible.
 
Checklist
  • If you are not the child or young person's parent/guardian/carer, please make sure you have permission from the above to nominate their child.
  • Address each section of the nomination form.
Prizes
  • The overall winner will win a $100 gift voucher and will be announced the week starting Monday 8 August.
  • The OSHC winners will win a $25 gift voucher and will be announced the week starting Monday 19 July.
