This Work Placement Register has been completed 4 weeks before the placement is expected.

I understand that I must complete a lesson check. A copy of the lesson check will be provided to the Work Placement SSO to initiate the process.

The Work Placement SSO will contact the employer.

DfE paperwork will be signed prior to commencement of the placement by Myself, Parent/Caregiver, Employer and School Principal.

My Parent/Caregiver will return the signed DfE paperwork within 4 days.

I WILL attend all days and times as agreed on DfE form. If I am ill and unable to attend a parent/caregiver must contact the employer and school prior to the agreed placement.
